New Patients
If you have an appointment, please arrive 15 minutes before your appointment to fill out forms or you can download the new patient forms. Bring insurance cards and a list of medications that you are currently taking. Co-payments need to be paid at time of service, and you are responsible for co-payments or deductibles that will not be paid by your insurance policy. We will assist you in filing your insurance claims.
We work with CareCredit…so everyone can enjoy a healthy and beautiful smile! It’s simple, you can apply at our office or online at CareCredit. CareCredit can be used to pay for co-payments, deductibles and treatment not covered by insurance, with no annual fees.
